On September 6th, all cadets who are returning to 75 Air Cadet Squadron will receive a document labelled: ANNUAL VALIDATION FORM.
This form will contain the personal information that we have in our files, we ask that you review all information and let us know if anything has changed by clearly marking it on the form.
PLEASE NOTE - SECTION 5: HEALTH INFORMATION
This form is required to be completed, signed and returned to administration by ALL RETURNING CADETS as soon as possible.
IN ADDITION to this validation form, any cadets who have indicated the following;
These cadets will be required to complete the additional forms above in order to continue their participation with the squadron.
** NOTE** If you completed the anaphylaxis forms last year but you were 13 years old at the time and have since turned 14 years old, the forms will need to be completed again.
These forms & all validation forms are required to be returned to the Administration Office by no later than THURSDAY SEPTEMBER 27TH, 2018. By this date, if any forms are still outstanding, those cadets will be restricted from participating in all cadet activities.
***Should you wish to participate in any of the optional activities during the month of September (i.e. Fall Gliding day, Seniors Weekend, etc.), your validation form must be submitted prior to being allowed to participate in that activity.
